Symptoms

  • Superficial abrasions may be asymptomatic
  • Deeper abrasions result in discomfort (foreign body sensation), pain and/or photophobia

Signs

  • Break in epithelial layer (superficial cells or full thickness epithelium)
  • Fluorescein staining

Etiology

  • Lens imperfections, foreign body, contact with fingernails, or improper insertion or removal technique

Prevalence

  • Rare

Differential Diagnosis

Management

  • Identify and remove cause
  • Cease lens wear for at least 24 hours up to 1 week for a full epithelial thickness abrasion
  • Consider topical prophylactic antibiotic
  • Consider topical lubricant. Consider pain management, as appropriate
  • Review in 1 day to 1 week depending on risk of infection
