Blebs (Endothelial)

Endothelial edema

Symptoms

  • None

Signs

  • Black spots which appear to be “holes” on endothelium when viewed with specular reflection
  • Intracellular edema of endothelial cells
  • Maximum response occurs 10-20 minutes after lens insertion.
  • Reduction in response occurs over initial weeks of lens wear.

Etiology

  • Local decrease in pH, associated with hypoxia and carbon dioxide build-up
  • Typically occurs in unadapted wearers. Degree related to oxygen transmissibility of the lens material

Prevalence

  • Occasional

Differential Diagnosis

Management

  • No action required
  • May consider refit into SiHy or high Dk GP lenses if response is severe
